  • Birmingham has a higher than average number of people with Diabetes, compared to the rest of the UK

  • If you’re from an Asian or Black community, you’re more likely to have Diabetes

Our new project aims to tackle these inequalities, so we are all able to live healthier lives.

Our project

Our Diabetes project is made up of face-to-face workshops. With the help of an expert lifestyle coach, you will have access to:

  • personalised plans for exercise, and a culturally appropriate nutrition plan, so you’re able to make the best choices for your health.

  • a physical activity programme

  • support for your emotional wellbeing

  • peer and group support

  • incentives for reaching your target goals

  • free access to leisure centres

Am I eligible?

There are a few ways to access this service. You can either:

  • Refer yourself, using the form below

  • Be referred from your GP

  • Be referred from a community group

How can I access this programme?

Our project is open to:

  • those aged 18 and over

  • those with Asian, Black African or Black Caribbean ethnicities

  • those with a BMI of over 27
