Our new project aims to tackle these inequalities, so we are all able to live healthier lives.
Our project
Our Diabetes project is made up of face-to-face workshops. With the help of an expert lifestyle coach, you will have access to:
personalised plans for exercise, and a culturally appropriate nutrition plan, so you’re able to make the best choices for your health.
a physical activity programme
support for your emotional wellbeing
peer and group support
incentives for reaching your target goals
free access to leisure centres

There are a few ways to access this service. You can either:
Refer yourself, using the form below
Be referred from your GP
Be referred from a community group
How can I access this programme?
Our project is open to:
those aged 18 and over
those with Asian, Black African or Black Caribbean ethnicities
those with a BMI of over 27
